If you want to surrender two (2) plates to the Dept. of Motor Vehicle, you need:

• Both plates
• Copy of the registration of the name of the person

 

IT’S EASY INC. FEE: $25.00 cash

You will receive back an FS-6 form which means the cancellation of your plates one day from the day we surrender the plates.

If you only have (1) plate, and wish to surrender that to the DMV:

Required:

• The (1) plate
• The name of whom the plates belong to
